It seems the accepted approach to working on a mental game is to learn how to cope more effectively. Most clients enter into our work together with this mindset because it is the one most often written about and used. The way most people go about fixing their performance issues is to find more coping mechanisms. The following is an excerpt from an upcoming book. The dialogue below takes place through Dans eyes (the client). I begin this portion with a question and Dans responses are in italics. He has just spoken about a number of his performance complaints:

Are you more interested in adopting more coping strategies to deal with all these performance issues you just told me about OR do you want to create a new approach that reduces the number of times they show up?

Dead silence. I thought the whole idea was to learn better coping strategies. I said.

Well, that would be the idea most people have but I dont believe its the optimal way to go about things. This is why Ive asked you the question. So, Im awaiting your answer.

Well, if I had a choice I guess Id like to create something new and reduce the number of times these things happenif I had a choice.

You do have a choice. David said.

Can you tell me why you asked that question?

Sure. Heres one of the first insights Id like you to consider. There is a difference between coping and creating and many people, out of habit or training, think coping is the way to go. When you look at what the word really means youll understand why I dont focus on it. For example, the synonyms for the word cope are: to manage, to get by, to struggle with and muddle through. Is that what youre striving for?

No, definitely not!

Well, thats what coping is!

When you explain it like that it sounds pretty average.

Learning to cope means you are keeping your chin above water and it reflects the mindset of just trying to get by. Its subtle and I want you to know you are capable of so much more than that. David said. End of excerpt.

I just received the following email from a client who read my earlier book Wired To Win and then took action to make an appointment. The original email is longer than what is presented here and Ive edited portions for the sake of space. He writes:

1. The best predictor of the well being of children involved in a divorce is the amount of conflict between parents. In mediation, conflicts are more likely to be resolved, resulting in a more peaceful post-divorce family life.

2. Mediation helps parents create truly thoughtful and child-focused parenting plans that are tailor-made to suit their childrens changing emotional, developmental and temperamental needs as well as the familys schedule.

3. By thinking through and discussing the parenting plan, possible problems can be identified and resolved before the final judgment is entered by the court.

4. Mediation creates opportunities for parents to work together and build on their strengths as they redefine the parental unit within the family. Parents who can model good conflict resolution skills for their children raise children with better conflict resolution skills.

5. A detailed parenting plan sends a message between the parents and others, including the children, new partners, school and court personnel that parenting is an important priority for both parents, even if one parent assumes more hands-on time with the children.

6. A detailed and thorough parenting plan pre-empts back and forth, He Said/She Said, arguments if differing views of the co-parenting history emerge.

7. Agreements, including modifications, create a record, or paper trail, of what was mutually agreed to when one or both parties were thinking more clearly about the issues involved in successful and co-operative co-parenting.

8. A detailed parenting plan sets forth a method to resolve differences without going to court in case you need to modify the parenting plan when things change, or if new partners, or reluctant children, want to unilaterally change the plan.

9. When co-parents deviate from the parenting plan and then fall into disagreement, a detailed parenting plan provides a useful backup plan until they return to mediation.

10. Mediation provides both parents with the opportunity to explore co-parenting issues with an objective third-party neutral who is a professional trained in childrens developmental needs and is knowledgeable about the research on childrens adjustment to separation and divorce.

Why do we root for Charlie, the womanizing, self-centered, hedonist? He’s played by Charlie Sheen, who had real-life practice for the part. How do we treat bipolar disorder? Specifically, how do we treat mania or depression associated with bipolar disorder? The treatment of these two clinical states is not the same.

The treatment of mania is dependent upon its severity and acuity. For mild to moderate mania, mood stabilizers such as lithium and valproic acid (Valproate) are still the standard of treatment and may be sufficient to contain the symptoms. Lithium starts to work after 10 to 14 days while valproic acid, about 7 to 10 days.

Also, recent studies have shown the effectiveness of atypical antipsychotics such as risperidone, olanzapine, and quetiapine even when used alone to treat the acute phase of bipolar disorder.

These drugs are relatively safe but they dont come without side effects. Nausea, vomiting, tremors, and dizziness during the initial phase of treatment are commonly experienced. The more serious side effects such as renal and thyroid problem from lithium, liver dysfunction and pancreatitis from valproic acid, and increased risk of diabetes and high cholesterol from atypical antipsychotics are uncommon. However, regular blood tests are required to monitor any abnormalities.

For moderate to severe cases, atypical antipsychotics such as risperidone and quetiapine should be added to the mood stabilizers during the acute phase. Once the illness has stabilized and the symptoms have subsided, then the atypical neuroleptics can be gradually tapered off. But the mood stabilizers should continue. Regardless of severity, patients usually do well on a combination of mood stabilizer and atypical antipsychotic during the acute phase.

What is the treatment for bipolar depression? In general, the mood stabilizers dosage should be optimized or if the patient is not on any medication yet, a mood stabilizer such as lithium should be started. Physicians should make sure that the medication maintains a therapeutic level. If not, the dosage should be adjusted. Moreover, possible precipitants such as stresses at home should be addressed.

If these measures dont help and the depression is so severe, an antidepressant with the least risk to induce mania such as bupropion should be added to the mood stabilizer. When the depression is resolved, then the antidepressant can be gradually tapered off because its prolonged use even in the presence of mood stabilizer can still induce mania.

When should the medication be discontinued? Bipolar patients have to continue taking the medication for several months even after they become normal. High relapse rate is common if medications are prematurely stopped. Also, for patients with multiple or difficult-to-treat episodes, they may need to take the medication for years or even for life to prevent recurrence.

Patients and their physicians should thoroughly discuss the risk and benefits of any treatment intervention. Knowledge about the drugs indication, side effects, and prognosis with or without treatment is a must.

Furthermore, it is crucial that bipolar patients should also receive individual psychotherapy to help them deal with the many personal and psychosocial issues they face on a daily basis. As you know, medication alone wont suffice to address financial problems, marital conflict, work issues, and prior abuse.

In summary, the combination of medication and psychotherapy is the best treatment for bipolar disorder.
