Dangerous Students Can Be Identified
STUDENT CRITICAL SYMPTOMS QUESTIONNAIRE
For grades 6 through 12 and 1st year college
Instructions for Administration
All questions are to be read aloud by examiner and answered YES or NO by student. Positive responses should be interpreted cautiously because every question can prompt an abnormal YES response. This is a PRELIMINARY effort to develop a valid test, but in its present stage of development, it can give only clues. The number of positive responses justifying further evaluation by a qualified psychologist has yet to be established. However, the examiner may give certain responses added weight, according to the examiners own best judgment, especially if experienced and if familiar with examinees history. Questions can be repeated upon request.
Before administration, put student at ease by explaining that this is a routine test to determine anything special that the school can do to make him/her comfortable. State that this questionnaire may not be necessary in his/her case, but that all students are given this opportunity, especially when new to the school, and that it is for their benefit.
Say to student, Here are some questions Im going to ask. Some may sound strange, but dont let that bother you. Remember that this questionnaire is given to many people, and everybodys background is different. Please answer YES or NO, whichever comes to you first. Try not to hesitate. Lets begin. This wont take long.

1. I have been beaten up.
2. I dont know if I want to be ignored or not.
3. It is possible to be controlled by somebody else or something else.
4. I daydream about getting even.
5. Very few people can understand me.
6. I lose the look on my face so nobody can tell what I am feeling or thinking.
7. Something bad happened to me which is my fault.
8. I have been insulted again and again.
9. If people knew my real powers, they would respect me.
10. There are secrets I will never share with anyone.
11. I have no reason to respect my teachers.
12. Some people do not deserve to live.
13. My mind has very special powers.
14. The person I share a secret with is an adult.
15. There are weapons in my home at least one.
16. I do not want to be like other people.
17. I have secrets that would make some people think I am crazy.
18. I daydream about paying back the people who hurt me.
19. I would feel better if some people left me alone.
20. There is no use talking to my mother or father.
21. The person I share a secret with is much older than I am.
22. Breaking things in a rage is not unusual for me.
23. I have a brother or sister who gets better treatment than I do.
24. I have rolled up in a ball on the floor a lot, or in bed.
25. It would suit me fine if I never had to go home.
26. Coming to school is very hard for me.
27. I hate for the other kids to see me naked in the locker room.
28. I think that all people are animals deep down.
29. When I am home, I rarely come out of my room.
30. There are secrets about me that nobody will ever know.
31. My favorite video or internet games are very violent.
32. Something inside me caused another person to do something bad to me.
33. I am not loved at home the way most kids are.
34. If people knew how important I am, they would admire me.
35. Things have been done to me that I can never tell about.
36. The way other students act is stupid.
37. There are people who secretly want to hurt me.
38. Some people are perfect.
39. Some people think I am weird.
40. My secrets could get me in terrible trouble.
41. I know that one day, I will get justice.
42. I have powers that people do not know about.
43. Most students are mean.
44. I am extremely brilliant, but others cannot recognize it.
45. I keep thinking about my private parts.
46. Words can hurt almost as much as physical pain.
47. I love movies and games where there is plenty of blood.
48. I try hard to avoid certain people.
49. I freeze in place and do not move for a while.
50. I have banged my head against the wall or the back of my bed.
51. I have thought of using a weapon on my enemies.
52. Some people can read your mind.
53. Some people are planning bad things for me.
54. I daydream or stare off into space a lot.
55. I forget what I am doing.
56. I have never told the truth about what somebody did to me.
57. I do not feel pain when something should hurt.
58. I feel afraid but do not know why.
59. Sometimes I have greater power than anybody.
60. I often feel guilty without knowing why.
61. I worry about my temper.
62. I try very hard to be perfect.
63. I have gotten so mad at people that I have worried about myself.
64. I share a secret with a particular person.
65. In school, I do not want to be part of any group.
66. I used to hate for certain people to give me a bath or a shower.
67. The groups in this school are stupid.
68. I feel very hopeless about my life.
69. There is a certain person who gives me a weird feeling.
70. I almost never feel guilty.
71. If I ever told the truth about what happened to me, somebody would get in trouble.
72. I can feel afraid, mad, and sad all at once.
73. I have felt like hurting myself on purpose.
74. Some people think I am a nothing.
75. I have done dangerous things, where I knew I would get hurt.
76. I believe strongly in the right to own a gun.
